==== Nixon's role reviewed ==== Decades later, a controversial quote attributed to John Ehrlichman, Nixon's domestic policy advisor, claimed that the war on drugs was fabricated to undermine the anti-war movement and African-Americans. In a 2016 Harper's cover story, Ehrlichman, who died in 1999, was quoted from journalist Dan Baum's 1994 interview notes: "... by getting the public to associate the hippies with marijuana and blacks with heroin, and then criminalizing both heavily, we could disrupt those communities. We could arrest their leaders, raid their homes, break up their meetings, and vilify them night after night on the evening news. Did we know we were lying about the drugs? Of course we did." The veracity of the quote was challenged by Ehrlichman's children, and Nixon-era officials. In the end, the increasingly punitive reshaping of US drug policy by later administrations was most responsible for creating some of the conditions Ehrlichman described. In a 2011 commentary, Robert DuPont, Nixon's drug czar, argued that the Comprehensive Drug Abuse Act had represented a degree of drug reform. He noted that the act had rolled back mandatory minimum sentencing and balanced the "long-dominant law enforcement approach to drug policy, known as 'supply reduction'" with an "entirely new and massive commitment to prevention, intervention and treatment, known as 'demand reduction'". == Indication == Opiates with opioid activity are mainly used for pain management with the premise that there will be benefits for both pain & function that will outweigh the risks to the patient. Another indication is symptomatic relief of shortage of breath, both in the acute setting (for example, pulmonary edema) and in terminally ill patients. In spite of scarce, often conflicting, evidence, at times studies showing no benefit at all, opioids such as codeine, hydrocodone, and hydromorphone have been traditionally used for treatment of acute viral cough (aka "acute bronchitis"), cough due to COPD exacerbation, chronic post-viral cough, chronic idiopathic cough, and cough from other causes. Given the abuse potential, the frequent GI side effects, and several studies showing no discernable benefit, recommendations are against use of opioids for cough in children. In spite of widespread use, the science supporting use of opioids for cough in adults is most notable for the small sample size, poor study design, and inconclusive results which suggest that there may be a small reduction in the amount of coughing when it is precisely measured. Actual evidence of patient-oriented outcomes, e.g. do the patients feel any better than when given a placebo, is elusive. The use of codeine as the "gold-standard" for which other drugs can be compared has been called into question, as showing that a drug is as good as, or at least not worse than, a drug with no benefit and only noxious side effects and a potential for abuse leaves much to be desired.
== Medical uses == In the United States, temozolomide is indicated for the treatment of adults with newly diagnosed glioblastoma concomitantly with radiotherapy and subsequently as monotherapy treatment; or adults with newly diagnosed or refractory anaplastic astrocytoma. In the European Union, temozolomide is indicated for adults with newly diagnosed glioblastoma multiforme concomitantly with radiotherapy and subsequently as monotherapy treatment; or children from the age of three years, adolescents and adults with malignant glioma, such as glioblastoma multiforme or anaplastic astrocytoma, showing recurrence or progression after standard therapy. Temozolomide is normally prescribed in cycles, with the patient taking the medication once a day for five days, then going 28 days without it. In the UK, NHS guidelines recommend that the drug is only used for six cycles. It is considered unusual for patients to continue taking the drug past 12 cycles. Temozolomide is also used to treat aggressive pituitary tumors and pituitary cancer.
